These forms are written for doctors and nurses. Nothing typed into a form is sent or stored anywhere. The filled form exists only on the paper you print, so file the printed copy in the case sheet. If you are a patient, the sheet written for you is linked under each pack.
Renal biopsy

Reports to check, papers to complete and what to keep ready at the bedside.
2. Consent for renal biopsy
What the biopsy is, the complications with their rates, and the signatures.
3. Renal biopsy specimen referral form
The histopathology requisition that travels with the sample: clinical details, treatment, laboratory data and serology.
4. Post-renal biopsy instructions
Bed rest, fluids, urine samples, the BP and pulse schedule, and when to inform the doctor.

How the forms work
- Calendars, clocks and tick boxes where a date, a time or a yes or no is needed. Anything left empty prints as a blank line.
- Signatures are taken on paper. Each form prints the lines to sign on.
- The clinical content belongs to the consultant named on the form. A change to a figure or an order goes through that consultant.
