Understanding fever

Why the day of fever decides the test

Each infection shows up in the blood at a different time. The dengue NS1 test works best in the first 5 days. Dengue IgM antibodies usually appear from day 5. Scrub typhus and leptospira antibodies also need about 5 to 7 days. A blood culture for typhoid finds the germ most often in the first week, and before any antibiotic. A Widal test on one sample has no value, and in the first week it is usually negative even in typhoid. Doing the right test on the right day saves money and gives a real answer.

Dengue: why the danger comes as the fever settles

In dengue, the risky phase usually starts around day 3 to 7, often just as the fever comes down. Fluid can leak out of the blood vessels, the blood thickens and the blood pressure can drop. That is why a person who "feels better because the fever has gone" still needs to watch for stomach pain, vomiting, bleeding, drowsiness and passing less urine. Most people with dengue recover with fluids and rest. Platelets are given only when the count is below 10,000 without bleeding, or when there is bleeding.

Fever in Telangana: what is common

Telangana reported 10,077 dengue cases in 2024 and 8,412 in 2025 (NCVBDC). Malaria is now uncommon in the state (248 cases in 2024), but most of those were falciparum, the more dangerous type. Chikungunya rose sharply in 2024. Scrub typhus is often missed: in one Telangana hospital series only about 1 in 7 patients had the typical black scab, and cases peaked in August. Typhoid in India no longer responds well to ciprofloxacin and similar medicines.

Which painkiller is safe in fever

Use paracetamol only. Ibuprofen, diclofenac, nimesulide, aceclofenac, mefenamic acid and aspirin can cause bleeding in dengue and harm the kidneys when you are dehydrated. Avoid them until dengue is ruled out. Steroids do not help dengue.
