There is no account, no tracking of your answers, and no lead form standing between you and the result.
They are screening tools, not diagnoses. Screening tools are built to be over-sensitive on purpose: a high result means look further, not that you have the condition. Where a tool has a poor positive predictive value, it says so on its own page rather than leaving you to assume the worst.
Some of them will tell you nothing is wrong. That is the point. A tool that always concludes you need an appointment is a booking form wearing a lab coat. These will genuinely tell you when your numbers look fine, and several of them do that for most people who use them.
Two of the standard sleep questionnaires, and two of the standard eating-disorder questionnaires, are copyrighted instruments that require a paid or written licence before anyone may publish them. Rather than reproduce them without permission, or quietly rename them, the sleep and eating sections use an unscored checklist of the same clinical features, with no score and no cut-off. It is less tidy and it is honest.
Bring the result. Print it or take a photograph of the screen. Nothing here is meant to be alarming on its own, and most of what these tools find is straightforwardly treatable once somebody has actually looked at it.
